SkinPen or RF Microneedling? Here's How We Decide

Both have needles. Both get called microneedling. Both live under the same heading on most med spa menus, which is exactly why patients walk in asking for "the microneedling one" and get surprised when we ask a follow up question.

They are not the same treatment. They work through different mechanisms, they suit different concerns, and they ask different things of your calendar. Choosing between them is one of the more genuinely useful conversations we have in a consultation, so here is how we think about it.

What they actually have in common

Both create controlled micro channels in the skin. Both rely on your own repair response rather than adding anything to your face. Both are typically planned as a series rather than a one time event.

That is where the overlap ends.

SkinPen: the repair response, without heat

SkinPen Precision creates precise micro channels that invite the skin into its natural remodeling process. No energy, no heat. The depth is adjusted by your provider based on the area and what we are treating.

SkinPen is the first FDA cleared microneedling device, and its clearances are specific: improving the appearance of facial acne scars in adults aged 22 and older across all Fitzpatrick skin types, and improving the appearance of wrinkles on the neck for Fitzpatrick types II through IV.

In practice, it is often where we start. Downtime is modest, it is comfortable, and it pairs well with almost everything else on the menu.

We tend to reach for SkinPen when: the concern is surface level texture, the look of acne scarring, or overall skin quality, and when someone wants to begin with the gentler end of the resurfacing spectrum.

Pixel8 RF: energy delivered below the surface

Pixel8 RF Microneedling pairs those micro channels with bipolar radiofrequency energy delivered into the deeper layers of the skin. The needle depth and the energy level are both adjustable, which is what makes it usable at one setting in delicate areas and deeper elsewhere.

Because it works below the surface rather than resurfacing the top of the skin, it is designed to stimulate collagen at a different depth than SkinPen reaches. That is the real distinction. Not stronger or better. Deeper, and doing a different job.

It is also the treatment where provider judgment matters most. Depth selection, energy settings, and knowing the anatomy underneath are what separate a well planned RF treatment from a careless one, which is why we review your full skin and health history before we treat and why this one is never booked off a menu.

We tend to reach for Pixel8 RF when: the concern is early laxity, enlarged looking pores, or texture that has not responded to surface treatments, and when someone is a good candidate after that review.

How we actually decide

The honest answer is that we do not decide from a menu. We decide from your skin. A few of the things that move the recommendation one way or the other:

What is actually bothering you. "My skin looks tired" and "my jawline is softening" are different problems with different answers, even though both sometimes get the same treatment recommended elsewhere.

Your skin history. Previous treatments, how your skin heals, whether you tend toward post inflammatory pigmentation, whether there is active inflammation that needs to settle first.

Your calendar. Recovery differs between the two, and it differs again depending on the settings we use. If you have something in three weeks, that changes the plan.

The season. We are heading into the lower sun months, which is the friendliest window for most resurfacing work. It is a real reason to start now rather than in April.

Whether one treatment is even the right question. Sometimes the answer is a series of one. Sometimes it is both, spaced out. Sometimes it is neither, and what your skin actually needs is a peel and a better home routine first.

Can you do both?

Often, yes, though not in the same session and not without a plan. Treating at two different depths across a series is a reasonable strategy for the right patient. It is also the kind of decision that should come out of a consultation rather than a menu, because sequencing and spacing matter as much as the treatments themselves.

How many sessions will I need?
That depends on the treatment and what we are addressing. Both are usually planned as a series. We will give you a realistic number at your consultation rather than an optimistic one.

Which one has less downtime?
Generally SkinPen, though the settings we use change the answer. We will walk you through what to expect for the specific plan we build.

Can I do either one if I tan easily or have deeper skin?
Often yes, and it is one of the first things we assess. Your provider will confirm what is appropriate for your skin at your visit.
